Hospice eligibility guidelines by diagnosis

Use these Medicare-aligned indicators to gauge whether a patient may be appropriate for a hospice referral. When you are unsure, call us — our team helps confirm eligibility and the right timing for care.

Neurological decline — Alzheimer's, dementia, Parkinson's
  • Functional Assessment Staging (FAST) at Stage 7 or beyond
  • Unable to ambulate, dress, or bathe without assistance
  • Urinary and fecal incontinence
  • Speech limited to six or fewer intelligible words per day
  • Supporting comorbidities such as recurrent UTIs, dysphagia, or frequent falls
Heart failure (CHF)
  • New York Heart Association (NYHA) Class IV symptoms
  • Ejection fraction of 20% or less
  • Treatment-resistant symptomatic arrhythmias or unexplained syncope
  • Optimally treated and not a candidate for further intervention
Pulmonary disease (COPD)
  • Disabling dyspnea at rest with poor response to bronchodilators
  • Hypoxemia at rest (pO₂ ≤ 55 mm Hg or oxygen saturation ≤ 88%)
  • Resting tachycardia greater than 100 beats per minute
  • Progressive decline with recurrent infections or ER visits
Cancer (oncology)
  • Palliative Performance Scale (PPS) score of 70% or below
  • Metastatic or progressive disease with declining function
  • Decision to forgo or discontinue curative treatment
General decline & adult failure to thrive
  • Palliative Performance Scale (PPS) score of 50% or below with multiple comorbidities
  • Unintentional weight loss greater than 10% over six months
  • Serum albumin less than 2.5 g/dL
  • Progressive dependence in activities of daily living
