Hospice care is delivered by an interdisciplinary team built around your loved one and your family, not a single provider working alone. Your own physician stays part of the plan alongside our hospice medical director, and the team adjusts as your loved one's needs change. You'll get to know these people, and they'll get to know your family too.

Meet your Interdisciplinary Care Team

Every patient is cared for by a team of specialists, each with a distinct role. Here's who you'll come to know.

01Physician & Hospice Medical Director

Your loved one's own physician is welcome to stay involved. Our hospice medical director oversees the clinical plan and helps manage complex pain or symptoms alongside them.

02RN Case Manager

Your most frequent visitor and primary clinical contact. Your nurse manages symptoms and medications and teaches your family what to expect along the way.

03Hospice Aide

A certified nursing assistant who helps with bathing, dressing, and daily personal care — often becoming a familiar, trusted face in your home.

04Medical Social Worker

Helps your family navigate insurance questions, advance directives, and community resources, acting as your advocate for the logistics of care.

05Chaplain

Offers spiritual support for any faith tradition, or none at all, and can coordinate with your family's own clergy if you'd like.

06Trained Volunteers

Provide companionship, a listening ear, and respite time so family caregivers can rest.
